Here is a case I have seen -
Admission on 15/3/2021
35 yr old female who is a daily wage labourer came with complaints of swelling in the left side of neck .she was apparently asymptomatic 3 yrs back then developed sudden onset of swelling of size 3x3 cm and 2 more swellings of size 1x2 cm started increasing since 2 years on left side of neck firm in consistency
C/o headache -dragging type associated with blurring of vision -nausea and vomiting ,more during mid day , decreased sleep since 15 days
No axillary and inguinal lymph nodes
No h/o fever ,cough ,wt loss ,oral ulcers ,white discharge ,dysperonea
K/c/o renal stones 4 yrs back
Disc prolapse 4 yrs back
18 yrs back she had joint pains she used herbal medication for 1 yr/3 per day she had similar complaints for 2 yrs then subsided after
Menstrual history
Age of menarche-13 yrs bleeds-3/28 days
Not associated with clots
No dysmenorrhea
General examination-
No pallor ,icterus,clubbing ,cyanosis.
Bp -110/70 mm Hg
PR - 88bpm
RR -22cpm
On examination
Localised swelling
3x3 cm
2 LYmph nodes -0.5x0.5
Soft in consistency ,movable ,discreate
Ascc with tenderness
No local rise of temperature
No skin lesion of lymph nodes
Not matted
Other group of lymph nodes- normal
CVS-S1,S2 heard, no murmurs
RS-BAE+
P/A - soft, non tender
CNS-NAD
